Benefits counseling and Medicare help
ElderSource (Area Agency on Aging for Northeast Florida)
Covers Duval, Clay, St. Johns, Nassau and Baker counties. Handles benefits screening, care coordination and referrals for seniors and caregivers. Search "ElderSource Jacksonville" for current contact hours.
Florida SHINE (Serving Health Insurance Needs of Elders)
Florida's free SHIP counseling program. Volunteers help you compare Medicare Advantage, Medigap and Part D plans with no sales pitch attached. Look up your local SHINE office through the Florida Department of Elder Affairs.
Duval County Council on Aging programs
Local aging services and senior program referrals for Jacksonville and Duval County; ask ElderSource for the current program directory serving this area.
Transportation, libraries and daily living
JTA Connexion paratransit
Door-to-door transit for riders who can't use a standard JTA bus route, including trips to medical appointments across Jacksonville. Check jtafla.com for eligibility and booking details.
Jacksonville Public Library branches
Branches across Duval County offer computer access, technology help and quiet space for paperwork; several host benefits counseling sessions and community events for seniors.
Clay County and St. Johns County senior centers
Both counties run senior centers with meals, fitness classes and social activities. Contact the Clay County Council on Aging or St. Johns County Council on Aging for locations near Fleming Island, Orange Park or World Golf Village.

Baptist Health senior services
Baptist Medical Center Jacksonville (Southbank), Baptist South and Baptist Beaches offer senior health programs and screenings. Confirm network status before choosing a Medicare Advantage plan.
UF Health Jacksonville senior services
Full-service hospital with geriatric care and specialty clinics serving Jacksonville and surrounding counties.
Ascension St. Vincent's and community clinics
Additional hospital and primary care options around Riverside and Southside; check with your plan for current in-network status.
Check your hospital before you switch plans
Networks shift every plan year. If you're on Medicare Advantage in Jacksonville, confirm Baptist Health or UF Health Jacksonville is still in-network before you enroll in something new.
